Building Bridges to the Future
The Building Bridges to the Future Program helps special education students with developmental disabilities establish and implement transition planning services to ensure their future. Students who participate in this program will achieve successful transition from school into the adult world of learning, working, socializing and living in the community. This will increase their options and achieve placement into adult services matched with their preferences and needs.
Park Lawn, Clearbrook and Seguin Services will establish a new position to partner with schools on outreach to students and parents. The Transition Outreach Specialist (TOS) will collaborate with the student, family members, school personnel and community provider agencies in the development and implementation of the transition process.
With the guidance of the TOS, parents will receive complete information about legal, medical and financial supports to ensure their child will successfully transition into adulthood.
Through project supports which include coaching and linkages to financial, medical and legal resources, parents and special education students with developmental disabilities, ages 18-21 years will achieve successful transition from school into the adult world of learning, working, socializing and living in the community. Students will increase their personal career choices and achieve placement into adult services matched with their preferences and needs.
